Huduma Poa Health Network Kenya: Who are we and What do we do?

Huduma Poa Health Network is a social franchise ran by Kisumu Medical and Education Trust-KMET through the support of USAID in partnership with PS-Kenya and the Ministry of Health. The franchise was officially launched on January 30th 2013.

The network is currently made up of 80 privately owned Health Facilities serving urban and rural undeserved population with 63% rural and 37% urban distribution.

Huduma Poa operates in 15 counties drawn from the former Nyanza, Western and Rift Valley Provinces.

The network aims at harnessing skills and energies of private health providers to integrate health services & ensure their facilities are a one – stop shop for the target undeserved communities.

To realize this, the Franchise builds capacity of healthcare providers through workshops, quality assurance and on job training based on Ministry Of Health Standard operating procedures to enable them offer:

  • Family planning method mix;
  • HIV counselling & testing;
  • HIV care and treatment
  • Prevention of mother to child transmission of HIV/AID
  •  Cervical cancer screening,
  •  Integrated management of childhood illnesses
  • and TB case detection, treatment and follow-up

The franchise has an established demand creation component comprising of 160 community health workers and 5 demand creation officers mandated to forge linkages between the facilities and target communities to promote demand for and access to the health services in line with Ministry Of Health’s community strategy.

Within a period of 2 years Huduma Poa has reached to 143,000 women of reproductive age with integrated Reproductive health services and information through:

Huduma Poa Service Delivery Points, Huduma Poa Days, Community Health Promotion Sessions & Outreaches.

KMET & Partners Bring on board community health workers in the fight against TB in Nyanza.

Latest statistics show that Kenya has a large TB burden ranked at 15 out of 22 countries in the world that contribute to 80% TB cases with 3million of such cases going undetected.
It is due to such alarming statistics that KMET and Population Service Kenya in collaboration with the Ministry of Health have opted to scale up services offered in the franchised clinics (Huduma Poa and Tunza) in Nyanza with an aim of intensifying TB care detection, diagnosis, treatment and follow up.
KMET Health Communication and Marketing coordinator Amos Onderi said community health workers (CHWs) have been included in the fight against TB since they are expected to play a critical role in TB control ranging from active TB care search, referrals to defaulter tracing at community level.
Amos added that this programme also brings in private facilities in TB case management and diagnosis given that private facilities attend to about 60% of clients.
In the first training for 87 CHWs from Nyanza at Royal City hotel in Kisumu, Siaya County TB and Leprosy Coordinator Mary Wambura said the CHWs will promote early TB diagnosis and therefore boost the cure rates.
KMET CEO Monica Oguttu expressed her confidence that TB mortality rates in Nyanza will reduce since the CHWs are going to be TB change agents in the community and encourage people to go for screening.
She urged the CHWs to always consult with the providers and together walk with the community to prevent late diagnosis in what she dabbed “Operation Linda Community.”

Huduma Poa HIV testing and counseling training.

KMET last week conducted an HIV Testing and Counseling (HTC) training for providers whose facilities are within the Huduma Poa franchise.
The main aim was to train participants who were drawn from 12 different counties on how to use and interpret results on the new HIV test kits before the current ones are completely faced out.
A number of cases have been reported of clients being given inconsistent results with regard to their HIV status a situation which has prompted the government to opt for new test kits which are time crucial and easier to interpret results.
Caren Omiya a lab technician, who was also a facilitator, said the new test kits are expected to reduce erroneous results given to clients hence improve on efficiency of testing services.
The peak of the training was during the practicum session at Kisumu District Hospital and Jaramogi Oginga Odinga Teaching and Referral hospital where one of the participants managed to counsel and test a street boy who after knowing his status decided to go back home.
Kisumu District Medical laboratory technologist, Samuel Onjoro, who was also a facilitator, stressed on the need for providers to practice ethics in counseling and testing clients for good results to be realized.
  Health Communication and Marketing Coordinator Amos Onderi urged providers to implement the lessons learnt during training to offer quality services.
Amos also informed the providers that the current HIV test kits are still in use until the new ones are supplied to facilities hence clients should not be turned away because of lack of new test kits.
In her remarks during the closure of training, KMET Executive Director, Monica Oguttu stressed to providers to record and manage data on HIV properly since data is the only indicator of progress.
She also called on the providers to improve on HIV counseling and testing services in antenatal clinics due to the low outcomes as indicated in such clinics.
Monica finally challenged the providers to go back, train others in the facilities and deliver quality services.

Child mortality rates within western Kenya and North Rift regions has been pointed out as reason enough for providers to implement Integrated Management of Childhood Illnesses, IMCI, within their facilities.
KMET Deputy CEO Sam Owoko called on providers to embrace IMCI since it’s a conceptual approach which offers simple and effective methods to comprehensively prevent and help manage the leading causes of illness and mortality in children below five years.
Sam also said that KMET is ready to partner with and provide loans to the private health facilities which have funding problems that reduce their capacity to offer quality healthcare to clients.
He further outlined the benefits the health facilities would accrue such as accreditation and quality standards through Safe Care.
“You can start from level 1 all the way to level 5 which is the highest and gives a facility international status,” he said.

The training brought together providers from Kisumu, Homa Bay, Siaya, and Nandi among other counties from around Western Kenya.
The providers were able to learn several aspects of child disease. From the signs and symptoms which present themselves.
The providers were involved in practicum sessions at the Mbale Rural Health Centre and Vihiga District hospital where they attended to inpatient and outpatient.
The facilitators, Charles Ngwalla, Kisumu county clinical officer, George Nyawalo, county clinical officer Siaya, Azenath Aluoch, county clinical officer, Homa Bay County were instrumental in aiding the training session. With vast experience in child diseases they were crucial as they imparted knowledge to the providers.


Siloam and Cherengany Health Facilities Crowned as Centres of Excellence

Two private health facilities working with the KMET Medical Credit Fund (MCF) have been crowned as centers of excellence in their counties for attaining high standards of quality health provision.

IMG_3259Siloam Hospital in Kericho County and Cherengany Nursing Home in Trans Nzoia County are now model centers for other health facilities after attaining Level 3 SafeCare standards.

SafeCare are defined tools used to address patient safety and are aimed at satisfying patients’ needs through improved hospital infrastructure, human resource, hygiene, and privacy in a stepwise approach.

SafeCare has 5 levels and the highest level that has ever been attained by facilities in the SafeCare program in Kenya is level 3.

The two health facilities embarked on the journey to quality in 2011 which has seen them move from the minimum quality requirements of health provision (SafeCare Level1) to SafeCare level3 with technical support from the KMET MCF team.

Speaking after the release of the assessment report for Siloam

Hospital, Kericho county Chief Executive of Health, Hellen Ngeno urged health facility owners to embrace quality control measures such as SafeCare so as to improve client satisfaction.

“I would also urge KMET and their partners in the SafeCare program to consider working with public facilities,” she added.

Access to affordable financing

Private facilities that partner with KMET in the SafeCare program also benefit from affordable financing plans in order to enable them invest further in expansion of their health services and quality standards.

Cherengany hospital has been able to buy a CT scan machine, equip a fistula unit with ward beds, equip an operation theatre with beds and construct a new maternity unit with a loan they secured through partnership with KMET.

Similarly Siloam Hospital has been able to expand their services and infrastructure and are currently constructing elaborate surgical & dental units and an outpatient department. Eearlier on in the year they launched a fully equipped renal unit with a capacity of 3.

Quality Director SafeCare (Amsterdam), Nicole Spieker who visited Kenya for the award that SafeCare has partnered with the National Hospital Insurance Fund (NHIF) to introduce quality standards in all facilities enrolled with NHIF in bid to strengthen Public Private Partnerships.
